FIRST-TIME APPOINTMENT POLICY:

You should arrive at least 15 minutes before your first appointment to fill out the necessary paperwork, which can also be filled out prior to your first session (forms available under the Forms & Referral tab). 

CANCELLATION AND NO-SHOW POLICY:

If you need to cancel or reschedule, a 24-hour notice is required to avoid a $50.00 late cancellation/no-show fee.

CHECK-IN:

Since all patients are seen by appointment only, we ask that you arrive on time to your appointment and check-in with the receptionist, if available. If you arrive15 minutes after your appointment time, we reserve the right to cancel and/or reschedule your appointment. Arriving 15 minutes late to your appointment is considered a late cancellation/no-show and a $50.00 late cancellation/no-show fee will be assessed. If you are running late, please give our office a call to let us know so that we can do our best to figure out a plan and see if your therapist is able to work with the remaining time.

FACILITY POLICY:

We ask all clients to respect others while at our facility and not cause any disturbances. We also ask that parents not allow their children to run up and down the hallways, or cause other disturbances in the office building, as this can be disruptive to other therapy sessions in progress and other surrounding businesses.

Please inform us of any special concerns that we should be aware of that will help us better serve you (and your family).
